
Now that you know what Self-Directed Support is you need to find out if you are eligible (if you don’t know what Self-Directed Support is see the section entitled “What is Self-Directed Support”).
To get Self-Directed Support you need to have an “Eligibility Criteria Assessment”. This is carried out by your local authority (council). To get an Eligibility Criteria Assessment you can contact your local authority yourself or get someone else to contact them on your behalf. This could be a family member, a carer, doctor, a local charity or anyone else you would like to nominate to do this for you.
